Best evidence summary for medication safety management for discharged patients

Abstract: Objective To systematically retrieve, appraise, and summarize the best evidence on medication safety for discharged patients, and to provide evidence-based reference for safe medication in this population. Methods The "6S Pyramid" evidence model was applied to systematically search decision support systems, guideline websites, professional society websites, and databases for guidelines and systematic reviews, with a search period spanning from the inception to October 14, 2024. Two researchers independently assessed the quality of the included literature, and evidence was extracted and summarized from studies that met the quality criteria. Results A total of 16 studies were collected, including 6 clinical guidelines and 10 systematic reviews. Seventeen pieces of evidence were summarized and categorized into three primary domains: hospital-based medication safety guidance, discharge transition strategies, and post-discharge monitoring interventions. Conclusion Healthcare professionals should recognize the pivotal role of hospital-based medication safety interventions. Communication at various transitional stages and post-discharge monitoring should be considered to establish a comprehensive medication safety management program, thus to enhance medication safety among discharged patients.
