Abstract: Objective To localize The Heart Disease Fact Questionnaire (HDFQ) and to provide an assessment tool for the knowledge of heart disease risk of type 2 diabetes patients in China. Methods The HDFQ was translated then back translation, cultural adjustment and pre-experiment were performed. A total of 408 patients with type 2 diabetes were surveyed with the translated questionnaire,and the reliability and validity were tested by SPSS 23.0 and AMOS 22.0. Results The Chinese version of HDFQ consisted of 26 items.The content validity index (I-CVI) of the items ranged from 0.750 to 1.000, and the content validity index (S-CVI) of the questionnaire was 0.938.Two factors were extracted by exploratory factor analysis,and the cumulative variance contribution rate was 84.9%. Confirmatory factor analysis showed that all the indicators reached the fitness standard, and the model fitted well.The total Cronbach coefficient of the questionnaire was 0.881; the sub-half reliability 0.843 and the retest reliability 0.867,and the decision values were all greater than 3.0.The Pearson correlation coefficient between each item and the total score ranged from 0.443 to 0.795. Conclusion The Chinese version of the HDFQ consists of 26 items in 2 dimensions, and has good reliability and validity. It can be used as a tool to evaluate and measure the knowledge of heart disease risk in Chinese type 2 diabetes patients.
