Abstract: Objective To construct an evaluation index system for safety management of the elderly in nursing home integrating pension service with medical service. Methods Based on the “2-4 model” of safety culture and behavioral safety, combined with Wuli-Shili-Renli system approach (WRS approach), the initial evaluation index system was established after literature review, expert group discussion and Delphi method then analytic hierarchy process was used to identify the weight of each index. Results Twenty-six experts completed two rounds of expert consultation. The effective recovery rate of the two rounds of inquiry was 85% and 100% respectively;the authoritative coefficient 0.875,and the coordination coefficient 0.220. The final index system consisted of 3 first-level, 13 second-level and 36 third-level indicators; the highest weight of the first-level was Renli（0.376）and the lowest Wuli（0.290）. Conclusion The evaluation index system for the safety management of the elderly in nursing home integrating pension service with medical service is established and it provides evidences for safety management and assessment in nursing home and civil affair administration department.
