Objective To investigate the effect of action research (AR) on nursing skill of intestinal stoma in training for surgical nurses in colorectal ward. Methods An AR-based nursing skill training of intestinal stoma was carried out from January to August 2019. After problem-identifying and an complete AR cycle including planning, action, observation and reflection, a systematic training integrating theory with practice was provided for nurses in colorectal ward. Nursing skill of intestinal stoma including theoretical knowledge, colostomy bag change and how to deal with complications induced by stoma was assessed and patient satisfaction was evaluated. Results After two cycles of training, nurses performed better in terms of theoretical knowledge, colostomy bag change and how to deal with complications induced by stoma and it was statistical significant (P<0.05). Conclusion Systematic and standardized training based on AR is conductive to the improvement of nursing skill of intestinal stoma and patient satisfaction.

Objective To summarize the best evidence regarding the prevention and management of postpartum hemorrhage, and to provide reference for clinical nurses. Methods Evidence-based nursing method helped to identify the problems then we searched articles pertaining to the prevention and management of postpartum hemorrhage with 6S evidence model. The literature included clinical guidelines, clinical decisions, expert consensus, and summary of evidence. Two researchers worked independently to assess the quality and extract data meeting quality standards. Results Nine pieces of evidence were included, with 1 best practice recommendations reports, 6 guidelines, 1expert consensus and 1 evidence summaries. Finally, 21 pieces of best evidence from 6 dimensions were summarized. Conclusion Clinical managers should strengthen the management of emergency treatment procedures for postpartum hemorrhage and the standardized training of clinical medical staff, improve the prevention and treatment of postpartum hemorrhage, and reduce the incidence and mortality of postpartum hemorrhage as much as possible.

Objective To summarize the best evidence of assessing sexual and intimate relationship for patients at the terminal stage of life and to provide reference for health care professionals. Methods We searched BMJ Best Practice, Cochrane Library, OVID, SUM Search, Turning Research Into Practice Database (TRIP Database), National Guideline Clearinghouse(NGC), National Institute for Health and Care Excellence(NICE), PubMed, Web of Science, Embase, Proquest, Medlive, CNKI, Wanfang Database, CBMdisc and VIP Journals of Chinese Science for evidence pertaining to assessment of sexual and intimate relationship for patients at the terminal stage of life. Two researchers independently assessed the quality of the literature and extracted data. Results Eight articles were enrolled, including 2 qualitative studies and 6 systematic reviews. A total of 30 pieces of evidence were extracted and integrated into 5 dimensions with 23 pieces of best evidence, including the importance, content, obstacle factors of evaluation, precautions and the strategies of improving sexuality and intimate relationship. Conclusion Health care professionals should pay attention to the assessment of sexual and intimate relationships in patients at the end of life. It is important to clarify assessment content, overcome the obstacles to the assessment, and provide more training of sexual and intimate relationship assessment. According to the implicit and introverted culture of China, health care professionals should make targeted evidence choice on the premise of respecting patients' willingness and privacy.

Objective To investigate the attitudes toward life-sustaining treatment in citizens and explore its influencing factors. Methods From June 2018 to January 2019, with a cross-sectional survey method, 932 Wuhan adult citizens were investigated by a general information questionnaire and Life-sustaining Treatment Questionnaire. Binary logistic regression analysis was used to analyze the influencing factors. Results Among the 932 citizens, 600(64.4%) had heard of life-sustaining treatment and 842 (90.3%) rejected it in case that they were seriously ill or in a coma. Binary logistic regression analysis showed citizens without terminal illness (OR=2.153, P<0.001) tended to accept life-sustaining treatment, while those with advanced age (50~59 years: OR=0.472, P=0.028; ≥60 years: OR=0.402, P=0.013), with education background of junior college or above (OR=0.526, P=0.009) and terminal illness (OR=0.542, P=0.037) tended to reject it. Conclusion In the study, 64.4% have heard of life-sustaining treatment and 90.3% tend to reject it among 932 citizens. Age, educational background and whether suffering terminal illness are the major factors influencing attitudes to life-sustaining treatment. Education on the limitations of life-sustaining treatment should be strengthened to help citizens understand its essence so to make rational choices and improve the quality of life.

Objective To investigate the status quo of dignity in breast cancer women receiving chemotherapy and explore its influence factors. Methods With cross-sectional survey method, breast cancer women receiving chemotherapy from 7 tertiary grade-A hospitals were investigated by demographic questionnaire, Patients Dignity Inventory, Distress Thermometer and Perceived Social Support Scale from April to June 2019. Multiple linear regression was used to predict the influencing factors of patient dignity. Results The total score of dignity was 38.21±12.98. Multiple linear regression showed that three variables, including treatment plan, psychological distress and social support entered into the regression equation (P<0.01), which could explain 41.6% of the total variance of dignity. Conclusion The dignity of breast cancer women receiving chemotherapy is at a slightly low level. Medical staff should pay more attention to patients with history of mastectomy, those psychological distress and those without more social support and take continuous intervention to encourage family members and friends to extend care and love to the patients, thus to improve their dignity.

Objective To explore the application of perineal assessment scale in perineotomy. Methods Four hundred puerperae were selected and divided into observation group and control group, with 200 cases in each group. In the control group, puerperae received traditional perineal care and in the observation group, perineal assessment scale was used in guiding perineotomy . Rate of perineal incision, perineal injury and maternal and neonatal outcomes in the two groups were compared. Results There were lower rate of perineal incision and perineal injury in the observation group, indicating statistical significance (P<0.05) and the healing of perineum in the observation group was better (P<0.05). Conclusion Perineal assessment scale can be used to guide perineotomy, which helps to lower the rate of perineal incision and adverse maternal and outcomes.

Objective To construct an evaluation index system for community nurses' knowledge, attitude and behavior of home visit to the elderly and provide theoretical basis for the assessment and improvement of community nurses' home visit. Methods After literature review and expert interview, the initial index system was constructed. Delphi method was used to conduct 2 rounds of consultation with 15 experts then the self-rating questionnaire was developed after index screening and revision. Results After 2 rounds of consultation, the evaluation index system for community nurses' knowledge, attitude and behavior of home visit to the elderly consisting of 3 first-level, 11 second-level and 73 third-level items was developed. The authority coefficient of two rounds of expert consultation was both 0.845 and effective response rate 100%. The coordination coefficient for three levels of index during the second round of consultation was 0.125(P=0.165),0.303(P<0.001)and 0.195(P<0.001)respectively. Conclusion The evaluation index system for community nurses' knowledge, attitude and behavior of home visit to the elderly is scientific, policy-oriented and specialized then can provide assessment tool to community nurses' core ability of home visit to the elderly.
