Objective To construct a first-aid skill training program for nurses in primary healthcare institutions based on the train-the-trainer (TTT) model, and to provide reference for the sustainability of first-aid skill training in primary healthcare institutions. Methods Taking TTT model as theoretical foundation, we initially constructed a pool of entries based on literature review and analysis of policies and guidelines. Using convenience sampling, 195 primary hospitals in 21 prefectural cities in Guangdong were selected from April to June 2024, and we conducted a survey on the current status of and demand for first-aid skill training to form a correspondence questionnaire for experts. From July to October 2024, two rounds of Delphi correspondence with 15 experts in the fields of first-aid training, nursing education, and nursing management were conducted to establish the final draft. Results The response rates of 2 rounds of consultations were 100%; expert authority coefficient was 0.830, Kendall' W coordination coefficients were 0.461 and 0.377, respectively (both P<0.001), and the coefficients of variation for the second round of correspondence ranged from 0 to 0.240. The two-tiered first-aid skill training program for nurses in primary healthcare institutions based on TTT model consisted of 3 first-level, 12 secondary-level ad 40 third-level indicators in the first-tier program and 3 first-level, 11 secondary-level ad 35 third-level indicators in the second-tier program. Conclusion The training program constructed in this study is scientific, practical and innovative, and can provide reference for the construction of primary first-aid training talent pool and the sustainable development of first-aid skill training.
