脑卒中患者跌倒恐惧干预研究的范围综述

Intervention programs for fear of falling in stroke patients: a scoping review

摘要: 目的 对脑卒中患者跌倒恐惧的干预研究进行范围综述,为医护人员制定和实施相关干预提供参考。 方法 按照范围综述方法学,系统检索PubMed、Embase、CINAHL、Web of Science、Cochrane Library、中国知网、万方、维普和中国生物医学文献数据库,检索时限为建库至2023年12月20日,对检索文献进行筛选。 结果 共纳入28篇文献。脑卒中患者跌倒恐惧的干预方案包括运动训练、虚拟现实技术辅助干预、认知训练、健康教育、机器人辅助训练,干预形式包括线下干预和线下线上结合干预,干预频率和时长根据干预内容和形式的不同存在差异,干预后患者跌倒恐惧心理有明显改善。 结论 目前国内外针对脑卒中患者跌倒恐惧的干预方案内容和形式较为丰富,但我国针对脑卒中患者跌倒恐惧的干预仍需进一步优化。未来可制定个性化的联合干预方案,深入探讨干预效果,促进患者早日康复。

Abstract: Objective To conduct a scoping review of intervention programs for fear of falling in stroke patients, and to provide reference for medical staff to formulate and implement relevant intervention programs. Methods Based on the scoping methodology, PubMed, Embase, CINAHL, Web of Science, Cochrane Library, CNKI, Wanfang Database, VIP Database and China Biomedical Literature Database were systematically searched from the inception to December 20, 2023. The retrieved studies were then screened and analyzed. Results A total of 28 articles were included. The intervention programs for stroke patients with fear of falling mainly included exercise training, virtual reality technology-assisted intervention, cognitive training, health education, and robot-assisted training, and the intervention forms included offline intervention and combined offline and online intervention. The frequency and duration of intervention were heterogeneous according to different intervention programs and forms, and the patients' fear of falling was significantly improved after intervention. Conclusion Currently, the content and forms of intervention programs for stroke patients with fear of falling are abundant at home and abroad, but the intervention for fear of falling in stroke patients in China still needs to be further optimized. In the future, personalized joint intervention plan should be developed to deeply explore the effect of intervention and promote the recovery of patients.
