慢性心力衰竭患者护理问题评估指标体系的构建

摘要： 目的 构建慢性心力衰竭患者护理问题评估指标体系。方法 基于奥马哈问题分类系统,通过文献回顾,查阅病例资料,半结构式访谈,结合疾病特点,应用Delphi法和层次分析法,确定慢性心力衰竭患者护理问题评估体系的内容及各指标权重并进行信效度检验。结果 专家的积极系数为100%;权威系数为0.876;一、二、三级指标的协调系数分别是0.341、0.353、0.238;评估表的Cronbach α系数为0.789,整体内容效度指数为0.970,各条目内容效度指数为0.860~1。最终形成4个一级指标、29个二级指标、138个三级指标的慢性心力衰竭患者护理问题评估体系。结论 慢性心力衰竭患者护理问题评估体系科学可靠,具有专病特色,对心力衰竭患者护理评估、提高护理服务质量有较好的指导意义。

Abstract: Objective To develop an evaluation index system of nursing problem for patients with chronic heart failure (CHF). Methods With Omaha problem classification system, after literature review, clinical data analysis and semi-structured interviews, combined with the characteristics of CHF, the content of the system and index weight of each item was identified through Delphi method and Analytic Hierarchy Process, then its reliability and validity were tested. Results The expert positive coefficient was 100%; authority coefficient 0.876 and expert coordination coefficients was 0.341, 0.353, 0.238 respectively for the three levels of indicators. The Cronbach's α coefficient of the scale was 0.789; the item content validity index ranged from 0.860 to 1 and the whole content validity index was 0.970. The final evaluation index system of nursing problems for CHF patients was formed with 4 first-, 29 second-, and 138 third-level indicators. Conclusion With the characteristics of CHF, the evaluation index system of nursing problem for CHF patients was scientific and reliable, which provides guidance for nursing assessment and the improvement of nursing service quality.
