老年口腔专科门诊护理质量敏感指标的构建

摘要： 目的 构建适用于老年口腔专科门诊的护理质量敏感指标体系,为加强口腔专科门诊护理质量管理、保障老年患者安全提供规范、可量化的依据。方法 以“结构-过程-结果”三维质量模型为指导,在文献分析、半结构式访谈、小组讨论的基础上,采用德尔菲专家函询和层次分析法,确定老年口腔专科门诊护理质量敏感指标及其权重。结果 本研究共有21名专家完成2轮函询,函询问卷的有效回收率均为100%,专家权威系数为0.873,一、二、三级指标的肯德尔和谐系数分别是0.245、0.407、0.395,差异均有统计学意义（P<0.05）。最终形成包括3个一级指标、12个二级指标、58个三级指标的老年口腔专科门诊护理质量敏感指标体系。结论 本研究构建的护理敏感指标体系具有科学性及临床实用性,符合口腔专科特色,对老年口腔门诊护理质量持续改进具有重要指导意义。

Abstract: Objective To construct a nursing quality sensitivity index system available for geriatric stomatological outpatient, so as to provide standardized and quantifiable basis for reinforcing nursing quality management of oral specialty outpatient care and ensuring elderly patient safety. Methods We employed the “Structure-Process-Outcome” three-dimensional quality model in the study. After literature analysis, semi-structured interview and group discussion, Delphi expert consultation and analytic hierarchy process were used to determine the nursing quality sensitivity index and the weight of each indicator. Results In this study, a total of 21 experts completed two rounds of correspondence questionnaires. The effective recovery rate of the questionnaires was 100% and expert authority coefficients 0.873. Kendall'W was 0.245, 0.407 and 0.395 respectively for the three levels of indicators, showing statistical significance (P<0.05). Finally, a nursing quality sensitivity index system for geriatric stomatological outpatient was formed, consisting of 3 first-level, 12 second-level and 58 third-level indicators. Conclusion With oral-specific characteristics, the nursing quality sensitivity index system established in this study is scientific and clinically applicable. This system plays crucial guiding role in continuous improvement of nursing quality in geriatric stomatological outpatient.
