摘要: 目的 采用Meta分析方法研究院前大量输入冷液体诱导亚低温治疗院外心脏骤停患者的效果。方法 检索PubMed、EMbase、CINAHL 、Cochrane Library、中国知网、万方、维普数据库中建库至2018年10月发表的有关院前大量输入冷液体诱导亚低温治疗心脏骤停患者应用效果的随机对照试验,并应用RevMan 5.3进行统计分析。结果 最终纳入9篇文献,其中,英文文献8篇,中文1篇,共4 027例患者。经院前诱导亚低温治疗组和常规救治组患者入院时的体温比较,差异有统计学意义(MD=-0.91,95%CI:-1.10~-0.71,Z=9.25,P<0.01)。2组出院时生存率比较,差异无统计学意义(RR=1.01,95%CI:0.92-1.12,P=0.81);2组神经功能情况比较,差异无统计学意义(RR=0.98,95%CI:0.86-1.11,P=0.70);2组心脏骤停再次发生率情况比较,差异无统计学意义(RR=1.18,95%CI:0.99-1.41,P=0.06);以及2组患者肺水肿发生率情况比较,差异无统计学意义(RR=1.12,95%CI:0.75-1.67,P=0.57)。结论 院前大量输入冷液体能明显降低患者入院时的体温,但其并不能改善患者的生存或神经功能情况。关于院前大量输入冷液体是否会增加肺水肿的发生率和患者心脏骤停再次发生率有待进一步研究。
