老年住院患者跌倒警觉度影响因素的Meta分析

老年住院患者跌倒警觉度影响因素的Meta分析

摘要

目的 本文旨在评价影响老年住院患者跌倒警觉度的因素,为老年住院患者的跌倒预防提供参考。方法 计算机检索中国知网、维普、万方数据知识服务平台、中国生物医学文献数据库、PubMed、Web of Science、Embase、Cochrane Library 8个中英文数据库,检索时限均为建库至2024年6月16日,搜集有关影响老年住院患者跌倒警觉度影响因素的研究。由2名研究者独立筛选文献,提取资料并评价纳入研究的偏倚风险后使用Stata 17.0进行Meta分析。结果 共纳入12篇文献,包括8 538例患者,Meta分析结果显示:跌倒警觉度与性别(女)[OR=1.378,95%CI(1.111,1.709),P=0.030],运动功能好 [OR=0.952,95%CI(0.921,0.984),P=0.001],跌倒恐惧[OR=0.382,95%CI(0.221,0.661),P=0.015],心理状态[OR=0.799,95%CI(0.711,0.897),P=0.017],高危药物使用[OR=1.464,95%CI(1.193,1.795),P=0.028],慢性病负担[OR=1.405,95%CI(1.189,1.660),P=0.037]有关,而年龄[OR=1.024,95%CI(0.994,1.054),P=0.637],认知功能[OR=1.043,95%CI(0.930,1.169),P=0.474]还有待进一步验证。结论 老年住院患者跌倒警觉度与女性、高危药物使用及慢性病负担呈显著正相关,而与运动功能、心理状态及跌倒恐惧呈负相关,年龄与认知功能关联未达显著性,提示临床需针对可干预因素构建多维度预防策略,并关注风险感知与行为管理的平衡。
