目的 本研究旨在对结直肠癌患者化疗期间的症状群相关文献进行范围审查,为症状群的探索与管理提供依据。方法 以范围综述指南作为方法学框架,系统检索中英文数据库中涉及结直肠癌化疗期间症状群的相关研究,检索时限为建库至2024年6月30日,对文献检索结果进行筛选及分析汇总。结果 本研究共纳入13篇文献,涉及12种症状群,其中消化道症状群、周围神经症状群、疲乏症状群以及心理症状群最为常见。症状群评估工具和分析方法存在差异,群内症状构成尚不稳定,化疗期间症状群呈动态变化,需结合不同化疗阶段症状群特点进行重点干预。结论 未来研究应重点开发标准化的多维度评估工具,结合多种统计方法分析症状群,并探讨其与生物标志物的关系。此外,还需开展多维度、多时间点的纵向研究,以提高结果的普遍性和可信度,从而为制定精准的干预措施提供依据。
Abstract
Objective To conduct a scoping review on literature on symptom clusters in colorectal cancer patients undergoing chemotherapy, and to provide reference for the investigation and management of symptom clusters. Methods Scoping review was performed in this study. A systematic search in Chinese and English databases was conducted to collect studies on symptom clusters during chemotherapy for colorectal cancer, with a retrieval time spanning from the inception to June 30, 2024. Results Thirteen studies were collected, identifying 12 distinct symptom clusters. The most frequently reported clusters were digestive symptom clusters, peripheral nerve symptom clusters, fatigue symptom clusters and psychological symptom clusters. There were differences in assessment tools and analysis methods of symptom clusters, and the symptom composition within clusters showing instability. Symptom clusters were observed to fluctuate dynamically throughout chemotherapy, underscoring the need to consider their stage-specific characteristics. Conclusion Standardized multidimensional assessment tools needs to be developed in the future, and multiple statistical methods should be considered to analyze symptom clusters and explore their correlations with biomarkers. Furthermore, it is necessary to conduct multidimensional and longitudinal studies at multiple time points to enhance the universality and credibility of the results, thus providing a basis for precise intervention measures.
